This form is to be used when Titans teams are requesting to affiliate a Titans player from another Titans team. This form can only be filled out by a Titans Head Coach or Manager.
- Prior to completing this form, please make sure you have followed GELC Regulation 8 - Affiliations Rules, and consulted with the parents/guardians of the player and the head coach of the player's rostered team. Rostered team games take precedence.
- When selecting a player, make sure you reference GELC Regulation 8 - Affiliation Rules and ALA Regulation 5A - Affiliations for all rules regarding player affiliations.
